If you've ever spent an afternoon on hold trying to figure out what's covered, that's the work your Navigator takes over. Most families pay $0 because it's covered by Medicare.

How do I know if my parent qualifies?

Three things: they have Original Medicare (not Medicare Advantage), they live in a state we serve (NY, NJ, CA, FL, MA, MD, or PA), and they live with one or more ongoing health conditions. Not sure? We'll verify it for free, with no commitment — just reach out.

For most families, $0. CareNav is covered under Medicare's Chronic Care Management benefit. If your parent has a Medigap supplement or Medicaid, that typically covers the rest. Without one, a standard Medicare Part B coinsurance may apply — and we'll tell you exactly what to expect before anything begins. No contracts, no hidden fees.

What if my parent has Medicare Advantage?

Right now we work with Original Medicare. If your parent has a Medicare Advantage plan, reach out anyway — the plans and states we serve are expanding, and we'll point you to other options if we can't help yet.

Is this the same as a private geriatric care manager?

The work is similar — coordination, advocacy, planning — but private geriatric care managers typically charge out of pocket. CareNav provides that coordination through licensed nurses and social workers, covered by Medicare, so most families pay nothing.
