Is CareNav right for your family?

CareNav is covered by Medicare for eligible families. Here's how to know, and what it costs.

Your parent is likely eligible if they:

  • Have Original Medicare (Part A & Part B).
  • Live in a state we serve: New York, New Jersey, California, Florida, Massachusetts, Maryland, or Pennsylvania.
  • Live with one or more ongoing health conditions — common ones include diabetes, heart disease, COPD, kidney disease, arthritis, and dementia.

A note on Medicare Advantage

If your parent has a Medicare Advantage plan rather than Original Medicare, we may not be able to cover services today — but reach out, because the states and plans we work with are expanding.

What it costs

  • CareNav is covered under Medicare's Chronic Care Management benefit.
  • If your parent has a Medigap supplement or Medicaid, that typically covers the remaining coinsurance — so most families pay $0.
  • Without a supplement, a standard Medicare Part B coinsurance may apply. We'll tell you exactly what to expect before anything starts.
