How it works

How CareNav works

Getting started takes one conversation. Here's the whole path, start to finish.

  1. 01

    Confirm your coverage.

    Tell us what's going on with your parent. We verify their Medicare coverage so you know up front that CareNav is covered, and we'll tell you plainly what (if anything) you'd pay. Most families pay $0.

  2. 02

    Meet your Navigator.

    You're matched with a dedicated Navigator — a licensed nurse or social worker. They and a CareNav clinician take the time to learn your parent's full picture: their conditions, their medications, their doctors, and how they want to live.

  3. 03

    Your Navigator gets to work.

    They build a plan and start handling the legwork — appointments, prescriptions, home care, benefits, paperwork — and keep you updated however you prefer (call, text, or email).

  4. 04

    They stay your first call.

    Whenever something comes up — a new diagnosis, a hospital stay, a question at 4pm on a Tuesday — your Navigator already knows the situation. No re-explaining. No starting over.

What it costs

Covered by Medicare. $0 for most families.

Covered by Medicare's Chronic Care Management benefit. With a supplement or Medicaid, usually $0. No contracts; cancel anytime.
